Plan Ahead
The steps that make a future crisis easier: who decides, what your loved one wants, and where the papers are. Tick them off as a family.
This is a guide to the official steps, not legal or medical advice. Only your ticks are kept, on this phone.

Before any form, the most useful step is a conversation: the kind of life your loved one wants, and the treatment they are comfortable with.
- Ask what a good day looks like for them, and what worries them about getting older or sicker.
- Ask who they would want to speak for them with doctors if they cannot. In an Advance Care Plan this person is called the Nominated Healthcare Spokesperson.
- Tell the rest of the family what was said.
